What Perennial does

Your checkup history, from your records

Ask when your last Pap test was or what your colonoscopy found, and get the answer from your own records in plain language. Dates and findings from reports kept in different places come together in one history.

Your past reports, requested

Triangle asks your doctors' offices for records via Carequality and TEFCA; the rest, such as a vaccine record from the pharmacy, you upload yourself. If a report seems to be missing, Perennial flags it and helps you ask for it.

Reminders in plain words

Word a reminder as you'd say it out loud, for example every October for a flu shot. It arrives in the app and as a notification, on the dates you pick.

Results you can follow over the years

Cholesterol, A1c and other results from your labs are laid out year by year, each with a plain-language explanation. Membership adds a licensed clinician's look over your whole medical history.

What makes Perennial different

The whole list in one place

Your colonoscopy report may sit with a gastroenterology practice and your mammogram with an imaging center, each in its own portal. Perennial brings the dates and findings from your records into one history.

Your dates, from your file

Without your records connected, ChatGPT, Claude or Grok has nothing that says when you had your last mammogram. Perennial looks it up in your own file.

Room for a parent too

Keep a parent's checkup history too, in a profile you run for them. Share it with family or a doctor, and choose who can see it.

Year-round, month to month

A card starts Perennial's 7-day free trial, and after that the price is $9 a month. You can cancel any time, and no drug maker, lab or hospital is paying for Perennial, only its members.

Who uses Perennial

The team behind Perennial

Perennial is a Triangle service, and Triangle (Triangle Health, Inc. on paper) has its base in Pasadena, California. Triangle grew out of Arun Verma's brain tumor diagnosis in 2023 and the research he did by hand afterward; Arun leads it as CEO, and his co-founders, Justin Balthrop and Jonathan Grubb, run it with him. Perennial uses what Triangle built to keep your own checkup history straight, year after year.

About Triangle

How Perennial works

  1. Begin with the trial

Open the 7-day free trial and set up a profile for yourself or for a parent you look after.

  2. Prove it's you

Put your name to the consent forms, pass an identity check and decide who may see the profile. A clinician meets you for an intake visit and signs the records request with you.

  3. Triangle gathers the dates

Triangle's requests travel the national networks for you, and whatever they miss, such as a pharmacy vaccine record, you upload.

  4. Ask and set reminders

Look over your health topics and medications, then ask when you last had each test and set reminders for the year ahead. Share with family, connect your own assistant if you like, and reach Triangle at support@trianglehealth.com whenever you need help.

Frequently asked questions

Who keeps Perennial, and what does it keep track of?

Perennial is Triangle's way of keeping your screenings and checkups straight, using your own records. Signing up creates a Triangle account.

What does Perennial cost, and can I stop?

Perennial's 7-day free trial begins once you've added a card. After that, Triangle charges $9 monthly for Perennial, and you may cancel in any season.

Is Perennial a substitute for my doctor?

No. Diagnosing, treating and prescribing sit outside what Perennial does; it offers research and educational support from Triangle. Which screenings you have, and when, gets settled between you and your clinicians.

Could ChatGPT, Claude or Grok tell me my screening dates?

Yes. Once https://app.trianglehealth.com/mcp is saved in your assistant's MCP settings, it can answer from the parts of your file you allow, such as your screening dates. Triangle publishes a walkthrough per assistant at trianglehealth.com/agents.

How does Perennial keep my history private?

Triangle meets HIPAA requirements and encrypts the records behind your checkup history, in transit and at rest. It never sells your information or uses it for advertising, and you choose everyone who can see your profile.

Will Perennial tell me which screenings I need?

It shows what your records say you've had, when, and what the reports found. Which screenings you need depends on your age, history and risk, and that's a conversation with your doctor. Perennial helps you arrive with the facts.

Can Perennial order or book a screening?

No. Your doctor orders screenings, and you book them the way you normally would. Perennial helps you know what you've had and set reminders for what you plan to do.

Will it find records from all my doctors?

Not every practice, imaging center or pharmacy can be reached through the national networks. A mammogram report or a pharmacy flu shot record that doesn't arrive can be uploaded as a PDF or photo, and Perennial marks what may still be missing.
